More on Part D: The 2027 Drug Tier Traps
Medicare prescription plans group medications into a 5-tier system. The higher the tier, the more you pay out of pocket. For specialized medications, you are exposed to 100% of the retail cost immediately on January 1st.
π’ Tier 1 & Tier 2: Low-Cost Generics
Standard generic medications (like basic blood pressure or cholesterol pills). These carry a low, stable, flat dollar copay (often $0 to $5).
π‘ Tier 3: Preferred Brand Name
Many plans switch your cost from a flat dollar copay to a percentage-based Coinsurance (often 20% to 25% of the full retail price). If a brand drug retails for $400, your cost instantly jumps to $100 per refill.
π΄ Tier 4 & Tier 5: Specialty & High-Cost Drugs
This includes complex brand names, biological treatments, and specialized injections.
The 100% Cash Trap: If your plan features the standard 2027 maximum drug deductible of $700, you must pay 100% of the retail cost for Tier 4 and Tier 5 drugs immediately at the start of the year. If your specialty medication retails for $1,200, you must hand over $700 cash for your very first refill before the policy pays a single dime.
π‘οΈ Your 2027 Safety Valve: The $2,400 Protection Wall
The good news is that federal consumer protections strictly limit your annual exposure.
Deductible Phase: You pay 100% of your retail drug costs up to $700.
Initial Coverage Phase: You pay standard copays/coinsurance until your total out-of-pocket spending hits $2,400.
Catastrophic Phase ($0.00 Cost): Once you hit that exact $2,400 limit, you pay $0.00 for your covered prescriptions for the rest of the calendar year.
(Note: This hard cap only applies to medications listed on your specific plan's formulary. Monthly insurance premiums and medical doctor copays do not count toward this limit.)
